This Customer Service Guarantee (CSG) waiver template can be used by carriage service providers (CSPs) to inform their customers of their rights under the Telecommunications (Customer Service Guarantee) Standard 2011 (No 2) and includes a CSG waiver form that customers can use to consent to waive their rights under the Standard.
How this document can help you
The Telecommunications (Customer Service Guarantee) Standard 2011 (CSG Standard) sets out minimum performance standards for eligible services. The standards apply to connection periods, rectification periods and customer appointments. Part 5 of the CSG Standard provides that a customer is entitled to waive such protection and rights, however the customer must be provided with information explaining those protections and rights and the consequences of the waiver.

Among other things, the CSG Waiver includes:
Provisions setting out a CSP’s contact details;
Provisions outlining the giving and withdrawal of a CSG waiver consent;
Provisions setting out a summary of a customer’s rights and protections under the CSG;
Provisions setting out the time frame as to when the waiver will take effect;
A CSG waiver form that enables the customer to provide written confirmation of that consent (including the date which the consent is provided).

Payment only upon wizard completion
If the wizard is not completed, you don't pay anything. Once you or your client completes the wizard, you will be notified by email and sent a request to pay our fee of $29.95 plus GST. Upon payment you will receive a report with the client's responses to the questionnaire together with the document that has been generated using your client's responses to the wizard.
