Direct Debit Request
This direct debit request template sets out standard terms governing carriage service providers (CSPs) who offer direct debit as a payment method for a telecommunications service.
How this document can help your clients
Clause 5.7 of the Telecommunications Consumer Protections Code C628:2019 (TCP Code) sets out direct debit payment requirements for carriage service providers (CSPs) offering direct debit as a payment facility. These requirements include various rights of the customer to readily cancel or verify a direct debit authorisation and refund procedures for incorrect debit payments as well as the right for a customer to change, stop or defer a debit payment. A direct debit request form is included in this document for clients who do not already have a form (if required). Alternatively, clients can refer to their own direct debit form.
What it includes
Among other things, our Direct Debit Request includes:
- Authorisation by a customer for funds to be debited from a nominated account by the CSP;
- Provisions setting out credit card payment processing fees and dishonour fees if there are insufficient funds in the nominated accounts on the bill payment due date;
- Termination rights of the customer and CSP of any direct debit arrangements; and
- Terms that confirm the right of a customer to revoke a direct debit arrangement;
- Other requirements of clause 5.7 of the TCP Code that apply to direct debit arrangements.

Payment only upon wizard completion
If the wizard is not completed, you don't pay anything. Once you or your client completes the wizard, you will be notified by email and sent a request to pay our fee of $29.95 plus GST. Upon payment you will receive a report with the client's responses to the questionnaire together with the document that has been generated using your client's responses to the wizard.
